dtcreate: Add missing prototypes and more (64-bit)

- Fix missing prototypes
- Fix some 64-bit related problems (XtVaGetValues)
- Fix crash on dtcreate startup in create_applicationShell1()
- Add XmeFlushIconFileCache() prototype from <Xm/IconFileP.h>
